Iowa DIA Entities Book (IFA)

Welcome to the Iowa Department of Inspections and Appeals (DIA) Health Facilities Division Entities Book. Here you will find a link to the Iowa Department of Inspections and Appeals’ directory of all entities licensed and/or certified to operate in the state of Iowa.

DIA’s Health Facilities Division is the designated state survey and certification agency responsible for inspecting and licensing/certifying various health care entities, as well as health care providers and suppliers operating in the state of Iowa. Entities subject to DIA’s oversight and regulation include assisted living programs.

For a complete listing of all certified assisted living programs in Iowa, please visit the DIA’s Health Facilities Division website to view its Entities Book. Remember that this directory lists all assisted living programs certified to operate in the state of Iowa, including market-rate as well as affordable assisted living facilities.
